Prescription Information

PRESCRIPTION INFORMATION / FORM

As stated in the Principles of Veterinary Medical Ethics of the AVMA,

  • Dispensing or prescribing a prescription product requires a valid Veterinarian-Client-Patient Relationship.
  • Veterinarians should honor a client's request for a prescription in lieu of dispensing.
  • Veterinarians should not allow their medical judgment to be influenced by agreement by which they stand to profit through referring clients to other providers of services or products.
  • Veterinarians may choose whom they will serve. Once they have started patient care, veterinarians must not neglect their patients, and they must continue to provide professional services until they are relieved of their professional responsibilities.

This means that if your veterinarian would dispense a prescription item to you, he/she should honor your request to write you a prescription to purchase that item from any source you choose, is not to direct that business to a supplier with which he/she may gain from a financial arrangement and he/she must continue to provide professional services to you.

When ordering RX items, which you will find in the the product description, you must submit a prescription form. This can be done by printing the attached form, filling it out and having your veterinarian's signature. You can mail it to Animart 900 Green Valley Rd., fax it to 920-885-2812. Your RX items will not be delivered without this form on file. Please note, if you are currently approved to receive RX products and have a form on file with us, you do not need to fill out another form. This form must be on file for each new RX product you wish to purchase, or for any expired RX forms we have on file. Please note in the special instructions when ordering how the form will be submitted so we can watch for it and process your order as quickly as possible.
